Killed in action from an incident on 04/28/1972 while performing the duty of front seat pilot of an U.S. Army AH-1G Cobra Helicopter. They were the Cobra wing ship on 1LT Charles O'Connell (Cobra lead) on a scouting mission. Took round in transmission, crashed and rolled over trapping pilot WO1 William A. Haines in front seat. Aircraft burned. Both Haines and his Pilot In Command, CPT Paul Martindale were killed.
